ONLINE ENROLLMENT APPLICATION INDEPENDENT STUDY PROGRAM

Each full unit subject is available also in half-units. Please refer to this document for full course listings. Specify whether you want the first half or the last half. For half of a one-unit subject, divide the price by two and add $25.00. Tuition must be paid in full before transcript of credit is sent.	If for sufficient reason a student drops the program within 90 days, we, upon written request of a counselor, will cancel on the following basis: If no lessons have been submitted, we will retain $20.00 for matric costs and $50.00 per subject for textbooks and guides; however, if the student has submitted lessons on any subject, full tuition will be charged and no refund made. Enrollments may not be transferred from one student to another.
Include a matriculation fee of $20.00 the first time a student enrolls; for subsequent enrollments of the same student, place a check mark in the "Added Subject" box (p. 6)—the matriculation fee is waived.	The minimum enrollment period for completion is 5 weeks per course consecutively; thus, 10 weeks for two, 15 weeks for three, etc. Maximum time is 1 year (renewal possible). Local school deadlines must be met within these guidelines.
