Print the application and complete the personal information section. Please print neatly.
Your counselor or principal must authorize your registration with his or her signature on this enrollment application
List the course(s) to be taken.
Your counselor or principal must fill in the spaces for completion date and final examination..
Mail the completed application with your check or money order, payable to American School (we cannot be responsible for currency sent by mail) to:
